PATH Membership Form

We are pleased that you are interested in joining PATH. Membership dues are $15.00 annually for an individual, $17.50 for a family, or $10.00 for a student. Please fill in and submit the following form. To complete your membership application, you will need to pay your dues either by mail or online. Instructions will be provided after you submit the application.
